Request For Tax Exemptible Receipt Guideline

　

Attention: If You Need The Tax Exemptible Official Receipt

Please inform us:

by contacting our office at Tel:04-2281058  OR

by Fax through Fax No:04-2281058  OR

by e-mail:admin@disabledpersonspenang.org  OR

by normal mail .

1. Please provide us the details of your donation for verification such as a copy of:

             1.1 Online Inter-Bank Transfer Receipt OR

             1.2 ATM Transfer Receipt OR

             1.3 Cash Deposit Machine Receipt OR

             1.4 Cheque Deposit Receipt (If deposited directly into our bank account) OR

             1.5 Any Other Type of Deposit Receipt

 Note: The copy of your transaction receipt could be a:

            - Smartphone Snap Shot,

            - Scanned Copy,

            - Printed copy,

            - PDF document, etc.,

 as long as details of your donation are clearly shown for verification.

　

2. Inform us the Name to be appeared on the receipt.

3. Inform us the Mailing Address for the receipt.

4. Contact number (optional).

5. E-mail address (optional).

so that we can send you the tax exemptible receipt.
